The plan's Accelerated Death or "Living Benefit"
pays 50% of benefi ts while still living if a physician certifies
the insured has 12 months or less to live. The remainder is paid
to the beneficiary after death.**
** If a reduction of benefit, due to age,
is scheduled to begin within one year, then the benefit will be
based on the reduced amount. Please note: Receipt of accelerated
death benefits may affect eligibility for public assistance
programs and may be taxable. Prior to applying for accelerated
death benefits, you should consult with the appropriate social
services agency and assistance should be sought from a qualified
personal tax advisor. Minnesota residents must be under age 65 to
apply. The Accelerated Death Benefit is not available to residents
of Massachusetts.

Your employees can contribute up to 75% of the
cost of their premiums. This applies if your firm employs 4 or
more eligible people and 75% of your employees participate in the
plan.

Fact #7
Anyone can be named a beneficiary, such as a
spouse, children, parents, etc. You can also name your alma mater
or a non-profit organization, such as the American Architectural
Foundation, as beneficiary. In addition, your beneficiary may be
changed at any time by writing to the AIA Trust. Payments for
losses other than loss of your life will be made to you. The
policy may also be assigned to a bank as collateral.

Fact #8
In cases of a covered accidental death, the plan
pays double the life insurance benefit. It also pays 100% of the
life benefit in the event of a covered non-fatal injury for the
loss of two or more limbs, the sight of both eyes, or loss of one
limb and sight of one eye. Accidental Death and Dismemberment
benefits pay one half the life benefit for the covered loss of one
limb or sight of one eye.***
*** Loss of limb means severance at or
above the wrist or ankle. Loss of sight means total and permanent
loss. To be a covered loss the loss must be incurred within 90
days of an accident that occurs while insured and not be an
excluded loss.

Life Exclusions and
Limitations
Benefits are payable in the event of your death
from any cause except suicide during the first 12 months of
coverage. However, if suicide is committed during the first 12
months, we will refund the premiums paid to the date of death.
(Missouri residents: Benefits will not be paid for suicide within
the first two years of coverage if New York Life can show that
suicide was intended at the time of application.)

AD&D Exclusions and
Limitations
Accident benefits will not be paid for any loss
which is due to one of the following exclusions:
- Suicide, attempted suicide or intentionally self inflicted
injury, while sane or insane (Missouri residents: Benefits will
not be paid for suicide within the first two years of coverage if
New York Life can show that suicide was intended at the time of
application);
- War, an act of war or an armed conflict which involves the
armed forces of one or more countries.
- A loss that is related to an Insured members duty in the
military, naval or air service of any country;
- A loss that is due to or related to an on the job accidental
injury, even if the loss is not covered under a Workers'
Compensation Act or similar act;
- A loss that occurs while Operating, riding or descending from
any aircraft except when riding as a passenger in a regularly
scheduled flight in a licensed, commercial, non-military
aircraft;
- Disease/Infirmity—A loss that is due to or related to
disease or bodily infirmity of mind or body, medical or surgical
treatment of such disease or bodily infirmity, or bacterial
infections, except infections which occur as a result of an: (1)
accidental cut or wound, or (2) accidental ingestion of
contaminated material.

Affordable Rates
Take advantage of the AIA-member only rates. Your
firm may choose from two benefit levels: either one times (Plan 1)
or two times (Plan 2) the employee's annual base salary (rounded
to the next higher $1,000). The maximum benefit is $50,000 for
Plan 1 and $100,000 for Plan 2.

You will be billed later once your application is
approved. When an employee enters a new age category, the premium
will change on January 1st, coinciding with or next following
their birthday. Premiums may be changed by New York Life on a
classwide basis on any premium due date or when benefi ts change.

Complete the Employer Participation Agreement and an Employee
Application for each employee.

Coverage starts on the first day of the month
coinciding with or next following the date the application is
approved by New York Life, as long as the premium is paid within
30 days of the effective date. To be insured, an employee must be
working full-time at least 20 hours a week on the effective date
or the date the premium is paid, whichever is later.

Important Notice
How New York Life Underwrites Your Request for AIA coverage.
Information regarding insurability will be
treated as confi dential. In considering whether the persons in
your request for insurance qualify for coverage, we will rely on
the medical information you provide, and on the information you
authorize us to obtain from your doctor, other medical
practitioners and facilities, and other insurance companies to
which you have applied for insurance and MIB, Inc. (Medical
Information Bureau). New York Life will not disclose such
information to anyone except those you authorize or where required
by law. We may make a brief report to MIB; however, we will not
disclose our underwriting decision. Information in our files may
be seen by New York Life and Plan Administrator employees, but
only on a "need to know" basis in considering your request. Upon
receipt of all requested information we will make a determination
as to whether your request for coverage can be approved.
MIB is a nonprofit, membership organization of
life insurance companies which operates an information exchange on
behalf of its members. When you apply for insurance or submit a
claim for benefits to a MIB member company, medical or non medical
information may be given to the Bureau, which may then be
furnished to member companies. Upon written request, MIB will
arrange disclosure of any information it may have about you in its
file. MIB's information office is P.O. Box 105, Essex Station,
Boston, MA 02112, telephone 617-426-3660.
If we can not provide the coverage you request
with standard rates, we will tell you why. In all cases medical
records information will be given to a medical professional
designated by you (except for certain types of medical record
information in specified jurisdictions you may choose to receive
such information directly). If you feel our information is
inaccurate, you will be given a chance to correct or complete the
information in our files.
For New Mexico Residents, in addition,
protected persons* have a right of access to certain confidential
abuse information** we maintain in our files and they may choose
to receive such information directly. You have the right to
register as a protected person by sending a signed request to the
Administrator at the address listed on the application. Please
include your full name, date of birth, and current address.
For U.S. residents, your request is handled in
accordance with the Fair Credit Reporting Act procedures. If we
can not provide the coverage you requested, we will inform you as
to when such coverage will be effective. Under no circumstances
will coverage be effective prior to this date. Payment of premium
contribution with your application does not mean there is any
insurance in force before the effective date is determined by New
York Life.
*"Protected person" means a victim of
domestic abuse who has notified us that he or she is or has been a
victim of domestic abuse and who is an insured or prospective
insured.
**"Confidential abuse information" means
information about acts of domestic abuse of abuse status, the work
or home address or telephone number of a victim of domestic abuse
or the status of an applicant or insured as family member,
employer or associate of a victim of domestic abuse or a person
with whom an applicant or insured is known to have a direct, close
personal, family or abuse-related counseling relationship.
